Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services. Based on identified need, the interdisciplinary team works with you and your family to create a plan of care and outlines the services you will receive. In addition to the list below, PACE also includes services deemed necessary by your team to improve and maintain your overall health.
